        Use this form to add a NEW record show date to the on-line listings. items are required. items are computer generated.
      To fill in, left-click the mouse in each input box, or press the TAB key until the cursor is in the box to be filled in. Type your information. Pressing the ENTER key will VERIFY each item and take you to the next. This is useful on the DATE item where the computer calculates the DAY. When you are finished, click the SUBMIT button at bottom.
